Will you be ready to complete?

 

Have you...

 ⁃ Made all payments to DDP, treatment, DMV or any fines?

 ⁃ Made an appointment for an evaluation with the treatment provider you've seen or will be seeing?

 ⁃ Notified the instructor if you have chosen an alternate provider?

 ⁃ Given in documentation of your BAC and 12 hour journal?

 ⁃ Notified your provider to enter your treatment completion date in the DMV IDS System- DO NOT call the DDP Office until this date has been entered.

 

 

If all requirements have been met....

you are ready to go to your local DMV Office to obtain your license.

 

 

If any one is unfilled, you will NOT be eligible for completion and

you should NOT contact the DDP Office or go to DMV Office.
